Opportunity:

Home Health RN (All Inclusive BR) 8hr or 10hr shifts (open to discussion)

Job Details:

* 8-hour or 10-hour day shifts from 8:00 AM to 4:30 PM (open to discussion)
* 40 guaranteed hours per week (hours not guaranteed during orientation week or holidays when unit is closed)
* 13-week contract
* Every other weekend required (open to every weekend)
* Up to every other holiday rotation required
* No on-call or callback responsibilities
* Start at the Home Health office and cover a 9-county radius in Northeast Wisconsin with an average of 75-100 miles of travel per day
* Conduct 4-6 patient visits per day with travel time included in shift length
* Must have own vehicle for travel between patient homes
* Care for a wide variety of patients ranging in age from newborn to senior population
* Provide services to patients following illness, new diagnosis, injury, or surgery with the goal of returning patients to prior level of function
* Work within a multidisciplinary clinical team including nursing, physical/occupational/speech therapy, social work, and CNA staff
* Perform hands-on skills including IV site care, wound care, wound vac, and ostomy care
* Must be comfortable working in all home types without special requests regarding environment
* Up to 50 pounds of lifting, pushing, and pulling during patient care may be required
* Utilize advanced technology including remote patient monitoring, 1-lead electrocardiogram, and video visits
* Solid color scrubs or business casual dress required
* Provided equipment includes laptop, corporate phone, and clinical bag with vital equipment and PPE
* EPIC experience required; point of care documentation experience required; OASIS experience strongly preferred
* Minimum 2-3 years of acute home health experience required
* Associate degree or certificate required; BSN preferred
* Required certifications: Current Wisconsin RN license or Compact, AHA BLS
