Opportunity:
PT - Physical Therapist
Job Details:
* Day shift hours from 8:00 AM to 5:00 PM
* Full-time position working 5 days, 8 hours per day
* Weekend requirement: Friday through Tuesday shift schedule
* No on-call requirement
* Floating required to Inpatient Rehab/Acute settings
* Uniform top provided by facility
* Blocked scheduling not accommodated
* Current New Mexico Physical Therapist licensure required
* BLS certification from AHA required
* Bachelor's, Master's, or entry level Doctorate of Physical Therapy degree from an accredited college or university required
* Proficiency with all modalities and equipment used by a Physical Therapist required
* Provide physical therapy services to referred patients including assessment, treatment plan development and implementation, follow-up, and discharge planning
* Perform comprehensive diagnostic evaluations to determine physical and cognitive abilities of inpatients and outpatients
* Develop and implement individualized treatment plans with measurable goals in collaboration with patients and families
* Maintain thorough documentation including initial patient evaluations, daily and weekly progress notes, and discharge summaries
* Participate in patient care conferences, family conferences, in-service programs, and departmental meetings
* Assist with coordination and supervision of Physical Therapy and Physical Therapy Assistant student programs
* Supervise Licensed Physical Therapy Assistants, Rehab Technicians, and Volunteers
* Assist with clinical aspects of the department including program development, department operations, and Performance Improvement activities
* Counsel patients, families, and involved individuals regarding patient evaluation, care, and treatment
* Initiate and complete discharge planning in a timely manner and provide effective education and training
