RN - Case Management

Job Details:

* Work Monday through Friday, 8:00 AM to 4:30 PM
* Coordinate management of care and ensure optimum utilization of resources, service delivery, and compliance with external review agencies
* Provide ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs
* Enhance quality of patient management and satisfaction to promote continuity of care and cost effectiveness
* Integrate functions of case management, utilization review and management, and discharge planning
* Serve as a patient advocate through resource utilization, discharge planning, and addressing holistic patient needs
* Ensure patient preferences are considered throughout care coordination and plan implementation
* Minimum 2 years of recent RN Case Management experience required, with experience in a LTAC or STACH setting
* Experience with MCG, discharge planning across acute care settings, and strong interdisciplinary team collaboration required
* BLS certification required (must be through the American Heart Association)
* California RN license required
