Open Position: RN - Case Management

Opportunity:

Case Management RN

Job Details:

* 13-week contract with ASAP start date
* 5x8 day shifts
* Guaranteed 72 hours per 2-week period
* Every other weekend required
* Block scheduling type
* Patient ratio of 25 patients per day
* Must have at least 1 year of acute care experience
* Discharge planning and care coordination experience required
* Strong preference for candidates with diverse case management experience including Med Surg, LLOS, and ED
* Will float between all areas of the hospital including two campuses less than 2 miles apart
* Work with patient age groups including neonates, adolescents, infants, adults, pediatrics, and geriatrics
* Interdisciplinary support includes social services
* Charting system: Epic (experience required)
* Scrub color: Any (no reddish orange permitted)
* Parking is facility paid
* Required certifications: BLS
* Washington state license required
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* All RTO must be submitted at time of submittal
* 2 references required covering 1 year within the last 3 years

Facility:

This facility is a Level II Trauma Center with a total of 571 staffed beds across two campuses located less than 2 miles apart. The facility supports a comprehensive case management team focused on care coordination and discharge planning within an acute care setting. The organization provides interdisciplinary support including social services and utilizes Epic as its primary charting system.
