Opportunity:

RN - Case Management

Job Details:

* Seasoned travelers only - first-time travelers are not accepted
* Must have a minimum of 2 years of Acute Care Case Management and Discharge Planning experience
* Hospital experience required - behavioral health experience does not apply
* Patient ratios: 1:25 on days and 1:25 on weekends
* Weekend rotation required (every other weekend)
* No floating expected
* California state license required - pending license accepted but must be licensed upon start date
* Dress code: Business attire or scrubs
* Charting system: Epic
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* RTO restrictions: No more than 7 days
* All RTO and specific scheduling requests must be submitted upfront during submittal
* Experience required in assessing patients, care coordination, discharge planning, and utilization review
* Ability to organize and participate in family meetings and contribute to treatment plan development
* Experience with HMOs, IPAs, and similar managed care organizations
* Knowledge of adoption/foster care, drug/alcohol abuse treatment options, patients experiencing homelessness, mental health history or crisis, advance directives, POLST, and protective services processes
* Must be able to independently arrange DME, SNF placements, and ambulance transport
* Computerized documentation required - Epic experience
* Interdisciplinary support available including physical therapy and social services
* Patient age groups served: Neonates, adolescents, adults, and geriatrics
* Required skills include: Acute Hospital, Admission Criteria, Benefits Eligibility, Care Coordination, CMS, Department of Health, Discharge Planning, Disease Management, DRG, Emergency Department, ICU, Med/Surg, Needs Assessment/Order DME, Plan of Care, Prior Authorizations, Surgical Services, and The Joint Commission/Core Measures/National Safety Goals
