RN - Case Management
Job Details:
* Seasoned travelers only - first-time travelers will not be considered
* No floating required
* Weekend rotation required to meet department needs (every other weekend)
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* Patient ratios: Days 1:25, Weekends 1:25
* Minimum 2 years of Acute Care Case Management and Discharge Planning experience required - behavioral health experience does not apply
* Hospital experience required
* California state license required; pending license accepted but must be active upon start date
* RTO restriction: no more than 7 days
* Charting system: Epic
* Dress code: Business attire or scrubs
* Experience with assessing patients, care coordination, discharge planning, and utilization review required
* Ability to organize and participate in family meetings
* Experience with HMOs, IPAs, and similar managed care organizations
* Knowledge of adoption/foster care, drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, advance directives, POLST, and protective services processes
* Must be able to independently arrange DME, SNF placement, and ambulance transport
* Computerized documentation required; Epic charting system
* Interdisciplinary support includes physical therapy and social services
* Patient age groups served: neonates, adolescents, adults, and geriatrics
* Required skills include: acute hospital, admission criteria, benefits eligibility, care coordination, CMS knowledge, discharge planning, disease management, DRG knowledge, emergency department, ICU, medical/surgical, needs assessment and DME ordering, plan of care development, prior authorizations, surgical services, and Joint Commission/Core Measures/National Safety Goals

The facility supports interdisciplinary collaboration through partnerships with physical therapy and social services teams. The organization maintains high standards for patient care and utilizes Epic for computerized documentation across its departments.
