Opportunity:
MSW or LCSW DAYS PART TIME
Job Details:
* Part-time position working daytime hours
* Schedule includes every Monday supporting the Huntington's Disease department and 2 additional days per week supporting the Multiple Sclerosis department
* No call required
* No weekend shifts required
* Business attire dress code required
* Master's level social worker required with ability to practice in California; clinical licensure (LCSW) preferred
* Minimum 1 year of experience required
* Outpatient experience required
* Experience navigating community resources, chronic and/or neurological illness, palliative care, and/or hospice preferred
* Familiarity with SSDI, IHSS, and similar social support programs in California preferred
* Experience managing complex patient care including behavioral/mental health needs, geriatrics, and patients with physical and cognitive impairment preferred
* Responsibilities include contacting and screening patients for Huntington's Disease, assessing socioeconomic constraints, providing support with SDI forms and IHSS forms, and scheduling appointments
* Training and education will be provided for professionals new to Huntington's Disease
* First-time travelers and local candidates welcome
* Required certifications: BLS, LCSW (California)
* Charting system experience required; Epic experience required
