Opportunity:

Case Management RN

Job Details:

* 5x8 day shift schedule
* 13-week contract
* Guaranteed 72 hours per 2-week period
* Every other weekend required
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* Float between all areas of the hospital including two campuses less than 2 miles apart
* Minimum 1 year of experience required; first-time travelers accepted
* Must have acute care experience and discharge planning/care coordination experience
* Strong preference for candidates with diverse case management experience including Med Surg, LLOS, and ED
* Patient ratios of 25-30 patients per day
* Block scheduling type
* Charting system: Epic (experience required)
* Scrub color: Any (no reddish orange permitted)
* Parking is facility paid
* Required certifications: BLS, Washington State License
* Required skills: Acute hospital care, care coordination, CMS, discharge planning, emergency department, ICU, med surg, needs assessment/DME ordering, and plan of care
* Patient age groups include neonates, infants, adolescents, pediatrics, adults, and geriatrics
* Interdisciplinary support includes social services
* 2 professional references required covering 1 year within the last 3 years

Facility:

This facility is a Level II Trauma Center with a total of 571 staffed beds across two campuses. The organization operates a comprehensive case management unit focused on care coordination and discharge planning to support patients transitioning to the next level of care within an acute care setting. The facility offers interdisciplinary support including social services and utilizes Epic for charting across all units.
