Job Details:

* Day shift position with a patient caseload of up to 25 patients
* Weekend requirement is every other weekend, Saturday and Sunday, with a compensatory day off during the weekday
* No floating expected
* Business attire or scrubs required
* 2 years of acute medical Care Manager experience performing Care Coordination and Discharge Planning required
* Experience with HMOs, IPAs, and similar organizations required
* Experience assessing patients, performing Care Coordination, and Discharge Planning required
* Proficiency with computerized documentation required
* Epic charting system experience required
* Skills required include: Acute Hospital, Admission Criteria, Care Coordination, CMS, CPT coding and billing, Department of Health standards, determining medical necessity per evidence-based guidelines, Discharge Planning, Disease Management, DRG, HIPAA guidelines, ICU, MS, Needs Assessment/DME ordering, Plan of Care, and SDU/PCU/IMC/Obs experience
* Patients served include adults and geriatrics
* California state license required; pending license accepted but must be active upon start date
* Travel experience required; first-time travelers accepted but must be screened
* RTO requests limited to no more than 7 days
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* 40 orientation hours required

This is an acute care facility offering comprehensive inpatient medical services across a range of specialties. The organization supports interdisciplinary care coordination and discharge planning services for adult and geriatric patient populations. The facility utilizes Epic as its electronic charting system and maintains high standards for documentation and compliance across its care teams.
