Opportunity:

REGISTERED NURSE (RN) - Field Home Health/Direct Patient Care

Job Details:

* Monday through Friday day schedule with on-call rotation participation required
* Field-based, direct patient care position requiring travel throughout Mesa County to patients' homes
* Travel to patients' homes to complete scheduled home health visits and perform comprehensive nursing assessments
* Provide direct skilled nursing care and interventions within the RN scope of practice
* Develop, implement, and update individualized plans of care based on patient needs
* Monitor patients during home visits and communicate changes in condition to providers and the interdisciplinary team
* Provide hands-on patient and family education regarding medications, treatments, disease management, and care needs
* Collaborate with LPNs, PTs, OTs, other clinicians, caregivers, and providers
* Complete documentation for each home health visit in the EMR (HCHB preferred)
* Active Registered Nurse (RN) license in the State of Colorado or active NLC compact license required
* Valid driver's license, reliable transportation, and auto insurance required
* Strong assessment and critical thinking skills required
* Ability to work independently in the field and travel between scheduled patient home visits throughout the workday
* Comfort with documentation and EMR systems required
* Home health experience preferred but not required
