Home Health PT - Medford OR and Surrounding Areas

Job Details:

* 8-hour day shifts
* 1:1 patient ratio with visits of up to 6 patients per day; 30 productivity points standard
* No weekend requirements
* No on-call requirements
* Minimum 1 year of skilled home health experience required
* Oasis experience required
* BLS certification required
* Charting system: EPIC/Oasis
* Any color scrubs permitted
* Traveler must have their own reliable transportation
* Occasional maximum patient visit radius of 50+ miles; system groups clients to minimize drive and windshield time
* Mileage reimbursement provided at IRS standard rate
* Facility may call off up to one shift per pay period (every two weeks)
* Compact license must be in hand at time of submission
* Skills required include: acute care, outpatient clinic, rehabilitation, skilled nursing facility, body mechanics, cervicalgia, low back pain, post-op surgical care, spinal conditions, joint replacements, cognitive disorders, neurological conditions including Multiple Sclerosis, Muscular Dystrophy, Parkinson's disease, post-stroke rehabilitation, spinal cord injury, traumatic brain injury, home exercise program development, and patient/family education
