Estimated Gross Weekly Pay: $3098.40

Opportunity:

PT - SNF & Home Health

Job Details:

* Work in a combined SNF and Home Health setting, approximately 50/50 split that may fluctuate
* Occasional coverage to Hastings or Grand Island for SNF or Home Health may be needed
* Schedule is 5 days per week, approximately 8 hours per day
* Guaranteed 36 hours per week
* Supervise 2 PTAs
* Productivity expectations: 80% for SNF and approximately 6 patients per day for Home Health (28 patients per week at 90% productivity if only in Home Health)
* Charting systems: Net Health for SNF and Homecare Homebase (HCHB) for Home Health
* Required certifications: BLS Certification
* New graduates are welcome to apply
* No on-call responsibilities

Facility:

This healthcare organization operates both skilled nursing facility and home health settings, providing comprehensive rehabilitative services across multiple care environments. The facility utilizes industry-standard software platforms to support efficient patient documentation and care coordination. A therapy scheduler and contact person are available onsite to support daily operations and provider needs.
