Opportunity:
LCSW / MSW - Case Management
Job Details:
* Variable 5-day workweek with department operations occurring 7 days per week
* Typical schedule is Sunday through Thursday, subject to change depending on staffing needs
* Days off will vary and are not guaranteed to be back-to-back, although every effort will be made to schedule consecutive days off when possible
* Caseload of 20-23 patients per day with a daily census of 25
* Serve adult and geriatric patient populations
* Conduct initial assessments within 24 hours of patient admission
* Responsible for coordination of discharge planning and all post-acute care needs
* Work closely with medical team, insurance case managers, and utilization management case managers to safely discharge patients to the lowest level of care possible
* Care management department consists of RNs and MSW/LCSWs all performing discharge planning
* Department also includes clinical coordinators who assist with tasks not requiring licensure, such as sending referrals via Epic, following up with facilities for bed assignments, and faxing clinical documents
* Unit-based position working with a variety of patients with varying diagnoses
* Must be proficient in Epic (required)
* Minimum 1 full year of acute care inpatient hospital experience required
* Minimum 2 years of overall experience required
* Travel experience required
* First-time travelers will not be considered
* Candidates with experience specialized solely in behavioral health or emergency department will not be considered
* Trauma Level I and Level II experience preferred
* Required certifications: BLS, LCSW or MSW
* Scrub color: Turquoise
* Maximum of 7 days requested time off
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day
* 2 references required, covering 1 year within the last 3 years
* Skills checklist must be completed and submitted with application
* All travelers are expected to float between all facilities within the state; refusal to float may result in termination of assignment and/or forfeiture of guaranteed hours

This acute care facility operates a comprehensive care management department staffed by RNs and licensed social workers focused on discharge planning and patient coordination. The department supports patients across adult and geriatric populations and works collaboratively with an interdisciplinary team to ensure safe, efficient transitions to the appropriate level of care.
