Opportunity:

RN - GI Clinic / Navigator

Job Details:

* Day shift hours from 8:00 AM to 5:00 PM
* Minimum 1 year of experience required
* Medical/surgical experience required; ambulatory experience is a plus and highly preferred
* Must have multiple sclerosis (MS) experience
* Work in an ambulatory clinic setting, seeing patients one day per week in clinic
* Serve patients ages 18 and above, including adults and geriatrics
* Work with one designated MD, managing 3-4 rooms per MD and PA
* Patient diagnoses include general GI, Crohn's Disease, and Inflammatory Bowel Disease
* Handle 10-30 phone messages per day and manage emails through MyChart EMR
* Follow up on lab results from off-site labs including Quest and LabCorp
* Administer B12 injections and assist with infusions
* Support patient discharge education, medication instructions, and follow-up coordination (MAs assist)
* Manage direct admits through MD RN Navigator authorization process
* Perform biopsy follow-up and prioritize care coordination
* Float required within same clinic among other navigators
* Skills required: adults, intramuscular, intravenous infusions, patient teaching, pharmacy calls/patient refill requests, subcutaneous, telephone triage, and minor injections (e.g., B12)
* Required certifications: BLS, CCRN
* EPIC experience required and must be documented in work history
* Two supervisor references required when applying
* California state license required and must be in-hand when applying
