Opportunity:

Home Health PT - Mt. Angel, OR and Surrounding Areas (Northern Territory; Woodburn, Mt. Angel, Aurora, Molalla, Gervais, Silverton)

Job Details:

* 8-hour day shifts
* No weekend requirements
* No on-call requirements
* 1:1 patient ratio with visits of up to 6 patients per day; 30 productivity points standard
* Facility can call off up to one shift per pay period (every two weeks)
* Minimum 1 year of skilled home health experience required
* Required certification: BLS/BCLS
* OASIS documentation required
* Epic charting system experience preferred; charting completed via EPIC Remote Client on laptop and Rover phone app
* Skills required include: Geriatrics, Home Health, OASIS, and Orthopedics
* Serves adult and geriatric patient populations
* Traveler is expected to have their own transportation
* Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day; system groups clients to minimize drive and windshield time
* Mileage reimbursement at IRS standard rate
* Dress code is business casual or scrubs; no jeans, no specific scrub color required
* Laptop and work cell phone provided for documentation and work communication
* Parking is free
* 16 orientation hours
* Interpretation services available
