Opportunity:
RN - Case Management
Job Details:
* Day shift position working 8:00 AM to 4:30 PM, Sunday through Thursday
* 40 hours per week guaranteed
* No on-call responsibilities
* RN Case Management (CM) certification required
* Inpatient case management experience required
* Experience with strike force roles for large health systems preferred, with ability to handle heavy case loads with minimal training
* Bachelor's Degree in Nursing (BSN) preferred
* Minimum two years of recent experience in case management or related field required
* Current BLS certification required
* Active RN license in state of practice required
* Lead coordination of patient care across disciplines, monitoring appropriateness and timeliness of care
* Ensure interdisciplinary care plans align with patient clinical course, continuing care needs, and covered services
* Discuss resource utilization, treatment plans, estimated length of stay, and discharge planning with physicians
* Focus on complex patients including frequent ED utilizers, chronic pain patients, substance abuse patients, and homeless patients
* Coordinate patient transfers to tertiary centers, including transfer of patient information for continuity of care
* Manage frequent emergency department visitors through focused study reviews of previous and current admissions
* Provide oversight and collect data required for regulatory and accreditation compliance
