Job Details:

* OASIS experience required
* Serve as Case Manager for assigned patients and actively manage caseloads and visit times in accordance with agency productivity standards
* Assess patient and caregiver needs utilizing the nursing process to provide quality care in accordance with evidence-based practice and state and federal regulations
* Develop and communicate a realistic plan of care for each patient, ensuring patients and caregivers are included in the care plan process
* Complete OASIS documentation in accordance with agency policies, procedures, and state and federal regulations, staying up-to-date on changes to documentation requirements
* Implement medical plan of care, nursing interventions, and procedures to ensure safe and effective quality patient care
* Maintain up-to-date High-Tech skills including IV use and maintenance, wound VAC, and complex wound care
* Work collaboratively with LPNs to ensure the plan of care is followed, delegate tasks, and ensure patient needs are met
* Evaluate implemented plan of care including identified problems, care provided, and patient responses to ensure overall quality of care
* Maintain accurate and up-to-date documentation of nursing care to ensure effective integration of information for the healthcare team
* Follow agency protocol with McKesson processes including locking down visits, follow-up notes, and visit completion
* Perform patient and caregiver education on medication side effects, treatment options, discharge instructions, reportable conditions, and ranges of services
* Identify and document interdisciplinary needs of the patient including physical therapy and social work, and communicate information to other disciplines in a timely manner
* Accurately and completely fill out agency, state, and federal forms including HHABN and Medicare Notice of Non-Coverage in a timely manner
* Carry out visits in a time-effective manner and communicate information to the Scheduler in a timely manner
* Assess patients' needs for HCA services and provide ongoing supervision in accordance with state and federal regulations
* Transmit patient incident reports to the QI Specialist in a timely manner as necessary
* Coordinate safe patient discharges and/or transfers and ensure all appropriate paperwork and patient information is processed accordingly
* Coordinate patient care with physicians, providers, team members, and vendors in support of the recommended plan of care
* Participate in monthly peer review audits and provide constructive feedback in a professional manner
* Proactively schedule, coordinate, and facilitate case conferences with the manager and team members to maximize effective communication
