RN - Home Health
Job Details:
* Day shift position working 8:00 AM to 5:00 PM
* Schedule consists of five 8-hour days per week
* Weekend rotation required every 4 to 6 weeks
* On-call required 1 to 2 times per month from 5:00 PM to 8:00 AM
* Holiday rotation required
* Case manage 30 home health patients with an average of 5 patient visits per day
* Provide skilled nursing care to patients in their home environment
* Common patient diagnoses include post-orthopedic surgery, CHF/COPD disease management, wound care, neurological diagnoses, genitourinary conditions, diabetes management, oncology, IV antibiotic teaching, medication management, venipuncture/phlebotomy, foley catheter management, and ostomy management
* Required skills include cardiopulmonary assessment, wound care, foley catheter management (male and female), venipuncture/phlebotomy, IV management, ostomy management, staple removal, pleural catheter management, neurological assessment, diabetes management, and CHF/COPD management
* Use MatrixCare EMR system along with iPhone, tablet, and INR machine
* May float to other office locations as needed
* Orientation includes EMR training and joint visit training with a preceptor RN
* Minimum 1 year of experience required; first-time travelers accepted
* Required credentials: RN licensure, BLS certification, valid driver's license, personal vehicle, and auto insurance
* Dress code: Business casual or solid-colored scrubs
