Opportunity:
RN - Case Management
Job Details:
* Day shift position working Tuesday through Saturday from 8:00 AM to 4:30 PM
* 40 hours per week guaranteed
* No on-call responsibilities
* RN Case Management (CM) certification required
* Inpatient case management experience required
* Experience with strike force environments such as KP or HCA California preferred
* Must be able to handle a heavy case load with minimal training
* At least two years of recent experience in case management required
* Current BLS certification required
* Bachelor's Degree in Nursing (BSN) preferred
* Current RN licensure in state of practice required
* Lead coordination of patient care across disciplines, monitoring appropriateness and timeliness of care
* Ensure interdisciplinary care plans are consistent with patient clinical course and continuing care needs
* Collaborate with physicians on resource utilization, treatment plans, estimated length of stay, and discharge planning
* Focus on complex patients including frequent emergency department utilizers, chronic pain patients, substance abuse patients, and homeless patients
* Coordinate patient transfers to tertiary centers including transfer of patient information for continuity of care
* Manage frequent emergency department visitors through focus study reviews of previous and current admissions
* Provide oversight and collect data required for regulatory and accreditation compliance
