Opportunity:
RN - Hospice
Job Details:
* Primary shifts are 8:00 AM to 4:30 PM
* 1-2 nights per week of overnight on-call from 4:30 PM to 8:00 AM
* Rotating weekends and holidays required
* Care setting is home health
* Guaranteed 36 hours per week for 12-hour shifts or 40 hours per week for 8 to 10 hour shifts
* Work week runs Sunday through Saturday
* Provide care to assigned clients in a caring, safe, and efficient manner according to location standards and individualized plans of care
* Demonstrate knowledge of hospice palliative care, hospice philosophy, and scope of services
* Recognize different cultural attitudes and values toward death and dying
* Perform initial and ongoing client assessments and coordinate with interdisciplinary team as needed
* Involve client and family in the creation of the plan of care
* Complete required documentation within appropriate timeframes
* Process physician services and orders and complete quality assurance audits as assigned
* Work independently with minimal supervision
* Must be at least 18 years of age
* Graduate of an accredited school of nursing required
* Minnesota nursing license required or ability to obtain a temporary license
* BSN not required
* Required certifications: BLS (AHA)
