Opportunity:

RN - Case Manager

Job Details:

* Minimum 3 years of acute care case management experience required
* Utilization management experience required
* Active California RN license required - no pending or temporary licenses accepted
* Required certification: BLS
* EPIC charting system highly preferred
* No weekend shifts required
* No floating required
* No on-call required
* No scheduling requests can be accommodated - must have full flexibility
* Patient age groups include adolescents, adults, and geriatrics
* Daily patient ratio of 15 patients
* Skills required include discharge planning, care coordination, utilization of InterQual criteria, utilization of Milliman guidelines, ICD-10 coding, DRG knowledge, and HIPAA compliance
* Flu vaccination required - no exemptions

Facility:

This facility is a well-established acute care hospital offering a comprehensive range of medical services to a diverse patient population. With a daily census of approximately 255 patients, the facility supports interdisciplinary care coordination through social services and transportation resources. The case management department works closely with clinical teams to ensure effective discharge planning and continuity of care for patients across multiple age groups.
