RN - Home Hospice

Job Details:

* Home hospice nurse role requiring travel between patient appointments using personal vehicle
* Mileage reimbursed at the Federal Rate
* Mobile position serving Buncombe County
* Patient ratio of 1:15
* Day shift team consists of 1 team manager per branch, 1 CNC, 3 RN case managers, and 2-3 PCTs
* Assess patients, provide pain management, and educate families
* Perform documentation required for hospice certifications
* Hospice experience as a case manager required; must be self-sufficient
* Familiarity with Home Care Home Base EMR required
* Active driver's license required
* Hospice experience required; home health experience preferred
* Potential for backup on-call, though not very likely
* Dress code: scrubs
* Required certifications: RN, CPR
