Opportunity:

PT - Physical Therapist

Job Details:

* 90% outpatient and 10% inpatient caseload
* Consult with patients to learn about their physical condition, symptoms, and review medical history and referrals from providers
* Diagnose movement dysfunction by listening to patients and observing them as they move
* Develop individualized treatment plans for each patient
* Coach patients and teach therapeutic exercises and stretches to improve their condition
* Use hands-on therapy techniques such as massage to ease patients' pain and promote healing
* Keep detailed records relating to patient conditions, care plans, therapies, and progress
* Report patient progress and current status to appropriate parties including family members and providers
* Collaborate with speech therapists and other physical therapists to achieve the best patient outcomes
* Help patients manage or lessen pain associated with degenerative conditions or terminal diseases
* Maintain professional communication with all members of the healthcare team
* Master's Degree from an accredited physical therapy program required
* Experience working directly with patients in a long-term care or hospital setting required
* Current Physical Therapist state license required
* Current Basic Life Support (BLS) certification required or ability to obtain within 6 months of hire
* Strong documentation skills and excellent verbal communication required
* Knowledge and skills necessary to provide care appropriate to the age of patients served
