Opportunity:

RN - Case Manager

Job Details:

* Minimum 3 years of acute care case management experience required
* Active California RN license required - no pending or temporary licenses accepted
* BLS certification required
* EPIC charting system experience highly preferred
* No weekend shifts required
* No floating required
* No on-call required
* No scheduling requests can be accommodated - must remain flexible
* Patient ratio of 15 patients per day
* Serves patient age groups including adolescents, adults, and geriatrics
* Skills required include discharge planning, care coordination, utilization of InterQual and Milliman guidelines, ICD-10 coding, HIPAA guidelines, and plan of care management
* Interdisciplinary support available includes social services and transportation
* 2 supervisor-level references from within the past year required at time of submission
* Maximum of 2 weeks pre-approved time off allowed; no time off within first 2 weeks of assignment

The facility supports a daily census of approximately 255 patients and provides interdisciplinary resources including social services and transportation support. The case management unit operates with a structured team environment focused on coordinating high-quality care for a diverse patient population ranging from adolescents to geriatrics.
