Opportunity:
Physical Therapist
Job Details:
* Work 5 days per week, 40 hours per week
* Provide physical therapy services in swing bed and outpatient settings
* Perform comprehensive physical therapy assessments and develop individualized, evidence-based treatment plans
* Supervise and coordinate care with physical therapy assistants (PTAs)
* Provide therapeutic interventions focused on mobility, strength, pain reduction, and functional independence
* Collaborate with interdisciplinary team members to coordinate patient-centered care and discharge planning
* Educate patients and caregivers on safe mobility techniques, equipment use, and home exercise programs
* Deliver rehabilitative services to home health and hospice patients with particular attention to the geriatric population
* Assess health needs and potential or existing hazards, and consult with or refer patients and families to appropriate resources as needed
* Work is performed in patient homes, office settings, and community environments
* North Dakota Physical Therapist license required
* Current driver's license required with personal vehicle available for travel to patient homes
* Minimum 2 years of previous experience as a Physical Therapist strongly preferred
