RN - Home Health/Hospice

Job Details:

* Monday through Friday, 8:00 AM to 5:00 PM shift schedule
* On-call required one weeknight per week and every 3rd weekend (scheduled and on-call visits)
* Every 3rd weekend requirement
* No holiday requirements
* Coverage area includes Nelson, Spencer, and southern Bullitt counties
* Clinicians are expected to call patients to schedule visit times
* Complete Start of Care, recertifications, and daily visits
* See pediatric and adult patient populations
* Home Care Home Base (HCHB) experience required - no exceptions
* Hospice experience required
* Valid driver's license and insurance required
* No point system for patient visits
* Any color scrubs acceptable
* Required certifications: BLS Certification
* Post-acute care setting

Facility:

This post-acute care organization provides home health and hospice services to patients across multiple counties. The facility supports clinicians with on-call supervisory availability and utilizes Homecare Homebase (HCHB) as its charting system. Home Health and Hospice Aides are available to support the care team in delivering quality patient-centered care in the home setting.

Nelson County, KY
