Job Details:

* Day shift schedule
* Plan, organize, and direct home care services to meet the needs of patients in their homes
* Manage a patient caseload in an effective and efficient manner
* Provide patient care independently while collaborating with other core disciplines in accordance with the patient's plan of care
* Complete initial and ongoing comprehensive assessments of patient needs at appropriate time points
* Identify and report problems, abnormal findings, or nursing diagnoses that require follow-up
* Implement plan of care in accordance with identified needs and mutually agreed upon goals
* Coordinate patient care with other disciplines and integrate input from care team members
* Perform ongoing nursing assessments and update plans of care as needed
* Accurately complete and document all pertinent patient information in accordance with established policies and procedures
* Counsel and educate patients and families in meeting healthcare and nursing-related needs
* Initiate appropriate preventative and rehabilitative nursing procedures
* Must have EPIC experience
* Mileage reimbursement provided in addition to base rate
* Minimum 2 years of recent experience in the field required
* Bachelor's Degree in Nursing (BSN) preferred
* Acute care facility or home care experience preferred
* Required certifications: BLS or CPR certification from an approved vendor
* Valid Driver's License required and must be at least 21 years of age or older
* Current automobile insurance and a reliable automobile required
* Current nursing license per state of assignment required
