Opportunity:

Physical Therapist - Home Health

Job Details:

* Day shift schedule
* Mileage reimbursement provided in addition to rate
* EPIC experience required
* Doctor of Physical Therapy (DPT) degree required if license obtained on or after January 2016, or foreign equivalent
* Physical Therapy licensure in all applicable states of practice required
* Valid Driver's License required and must be at least 21 years of age or older
* Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification required from an approved vendor
* Minimum of two years recent experience in the field required
* Complete evaluations to ensure quality standards and address assessment areas requiring attention
* Use appropriate clinical techniques and tools to establish individualized treatment plans for patients
* Provide therapy interventions using appropriate treatment protocols and techniques
* Establish individualized care and discharge plans and ensure plans are communicated to appropriate parties
* Complete comprehensive and accurate clinical documentation within established deadlines
* Supervise assigned assistants
* Serve as co-leader in an interdisciplinary approach to patient groups
* Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions
