Physical Therapist

Job Details:

* Day shift schedule, Monday through Friday, 5 x 8-hour shifts
* No weekend requirements
* No on-call responsibilities
* Mileage reimbursed at government rate
* Must have knowledge of HCHB and OASIS platforms
* Must have EPIC experience
* Minimum 1 year of home health experience required
* Minimum 2 years of recent experience in field required
* Doctor of Physical Therapy (DPT) degree required if license obtained on or after January 2016
* Physical Therapy licensure in applicable state of practice required
* Valid Driver's License required and must be at least 21 years of age
* AHA-BLS or AHA-CPR certification required
* Complete evaluations to ensure quality standards and address assessment areas requiring attention
* Use appropriate clinical techniques and tools to establish individualized treatment plans for patients
* Provide therapy interventions using appropriate treatment protocols and techniques
* Establish individualized care and discharge plans and communicate plans to appropriate parties
* Complete comprehensive and accurate clinical documentation within established deadlines
* Supervise assigned assistants
* Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions
* Serve as co-leader in interdisciplinary approach to patient groups
