Opportunity:

Occupational Therapist - Home Health

Job Details:

* Day shift, Monday through Friday, 8-hour shifts
* Provide in-home occupational therapy services to patients throughout the community
* Complete evaluations to ensure quality standards and address assessment areas requiring attention
* Use appropriate clinical techniques and tools to establish individualized treatment plans for patients
* Provide therapy interventions using appropriate treatment protocols and techniques
* Establish individualized care and discharge plans and ensure plans are communicated to appropriate parties
* Complete comprehensive and accurate clinical documentation within established deadlines
* Supervise assigned therapy assistants
* Serve as co-leader in interdisciplinary approach to patient groups
* Home health experience is preferable but not required
* Must be proficient with HCHB and EPIC systems
* Mileage reimbursement provided in addition to base rate
* Master's degree in Occupational Therapy or Doctor of Occupational Therapy (OTD) required if license obtained on or after January 2007
* Occupational Therapy licensure in all applicable states of practice required
* CPR or BLS certification from approved vendor required
* Valid Driver's License required and must be at least 21 years of age or older
* Minimum two years of recent experience in field required
