Opportunity:

RN - Case Management

Job Details:

* Seasoned travelers only - first time travelers are not accepted
* Must have minimum 2 years of acute care case management and discharge planning experience
* Hospital experience required - behavioral health experience does not apply toward experience requirement
* Patient ratios: 1:25 on days, 1:45 on weekends
* Weekend rotation required to meet department needs (every other weekend)
* No floating expected
* Business attire or scrubs required
* California state license required - pending license accepted but must be licensed upon start date
* Charting system experience required - Epic
* Manage a caseload of up to 25 patients daily
* Patient population includes neonates, adolescents, adults, and geriatrics
* Experience required in assessing patients, care coordination, discharge planning, and utilization review
* Ability to organize and participate in family meetings and contribute to treatment plan development
* Experience with HMOs, IPAs, and similar organizations required
* Knowledge of adoption/foster care, drug/alcohol abuse treatment options, patients experiencing homelessness, mental health, advance directives, POLST, and protective services processes
* Must be able to independently arrange DME, SNF placements, and ambulance transport
* Required skills include: admission criteria, benefits eligibility, care coordination, CMS, Department of Health standards, discharge planning, disease management, DRG, emergency department, ICU, MS, needs assessment, ordering DME, plan of care, prior authorizations, surgical services, and Joint Commission/Core Measures/National Safety Goals
* Interdisciplinary support includes physical therapy and social services
* Computerized documentation required
* 2 professional references required covering 1 year of experience within the last 3 years
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* Maximum of 7 days of requested time off
* Skills checklist must be completed and submitted with application
