Opportunity:

RN - Home Health

Job Details:

* Work 8-hour day shifts with weekend rotation
* Weekend shift hours are 8:00 AM to 5:00 PM
* Included in holiday rotation
* No on-call requirements
* Cover Nelson, Spencer, and southern Bullitt counties (no mileage pay)
* Case manage approximately 30 adult patients per day
* Complete 6.5 points per day including start of care, recertifications, and daily visits
* Home Health only - does not cover Hospice
* Must have Home Care Home Base (HCHB) experience and background in home health - no exceptions
* Must have a minimum of 1 year of experience on a Med/Surg floor in a hospital setting
* Must be licensed in the state of work
* BSN not required
* Required certification: BLS (AHA)
* Charting system: Homecare Homebase (HCHB)
* Phone and tablet will be provided
* Scrubs required with no color requirement
* 2 weeks of orientation with a few visits alongside a work coach
* Clinical manager or educator available 24/7 for emergencies or 8:00 AM to 5:00 PM for standard support
