Physical Therapist (PT) - Skilled Nursing Facility

Job Details:

* Perform thorough evaluations to assess patients' physical abilities and develop personalized treatment plans
* Provide physical therapy interventions to improve strength, mobility, and functional abilities
* Educate patients and their families on therapeutic exercises, safe movement techniques, and home modifications
* Collaborate with an interdisciplinary team including occupational therapists, nurses, and physicians to coordinate care and optimize patient outcomes
* Monitor patient progress and adjust treatment plans as necessary to ensure the best possible outcomes
* Maintain accurate and timely documentation of patient assessments, progress notes, and treatment plans in accordance with facility policies
* Ensure care plans align with regulatory requirements and best practices in physical therapy
* Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program required
* Valid Physical Therapy license required
* Experience working in a skilled nursing facility or similar healthcare setting preferred
* Knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment required
* Ability to work both independently and collaboratively in a team environment
