Opportunity:
RN - Case Management
Job Details:
* Monday through Friday shifts from 8:00 AM to 4:30 PM (7.5-hour shifts with a 1-hour lunch break)
* Breaks are per facility policy with management supportive of taking a full lunch break
* Weekend coverage is occasionally required (rare and requires manager approval) with potential to cover multiple units
* Holiday coverage is occasionally required (rare and requires manager approval)
* BSN required
* Must have 1 year of inpatient case management experience
* Prior critical care experience preferred
* Required certifications: BLS, PRI (Patient Review Instrument), and SCREEN Training Certification
* PRI and SCREEN certifications can be completed online through the School of Albany (approximately $300 per certification)
* Must have knowledge of discharge planning and making referrals
* Must be able to function independently and work collaboratively with an interdisciplinary team and patients for discharge planning
* Expected to round on the unit with physicians and the interdisciplinary team
* Must demonstrate a positive attitude, critical thinking skills, and strong time management skills
* Will typically work on a Medical-Surgical or Telemetry unit, splitting the floor with a Social Worker
* Caseload typically ranges from 17 to 25 patients on standard floors and 12 to 17 patients on pilot floors
* Must be able to manage a caseload of at least 20 patients
* Common job duties include discharge planning to home, community, or SNF, obtaining housing, completing shelter packets for homelessness, hemodialysis referrals, and KCI referrals for wound vacs
* Covers inpatient areas following Internal Medicine and Family Medicine floors
* Preferred knowledge of CarePort, MCG, and/or InterQual; Utilization Review experience preferred
* Charting system: Allscripts
* Dress code: Business casual or any color scrubs except Royal Blue or Maroon (neutral colors preferred); lab coat provided
* Department-specific orientation includes 2 days of hospital orientation with EMR training and 1 to 2 weeks with a preceptor
Facility:
This acute care facility is a Level 2 Trauma Center offering comprehensive inpatient services across Medical-Surgical and Telemetry units. The facility supports an interdisciplinary approach to patient care and provides a collaborative work environment where Case Managers work alongside Social Workers, physicians, and other healthcare professionals. The organization utilizes Allscripts as its electronic medical records system and provides departmental resources and orientation to support new team members.
