Opportunity:

Care Coordinator

Job Details:

* Coordinate care and services for assigned patients during an episode of illness or treatment in collaboration with patients, families, physicians, patient care teams, and payors
* Utilize advanced clinical skills to facilitate care through assessment, planning, intervention, and evaluation principles of the nursing process
* Promote and evaluate effective utilization of resources using current clinical knowledge and awareness of community services
* Evaluate and analyze quality and utilization cases, admission and discharge criteria, observation and one-day stays, readmissions within 30 days, Medicare, and diagnosis related criteria
* Participate in ongoing outlier review
* May be assigned to the Emergency Department as needed
* Advocate for patients and families throughout the entire episode of care
* Maintain availability to patients and families as a resource to facilitate communication among providers and monitor services rendered
* Remain involved until the patient is discharged or appropriately transferred to an acute care setting or alternate level of care
* Initiate appropriate clinical pathways based on admission diagnosis and serve as a resource person for utilization management purposes
* At least one year of recent Case Management experience in an acute care setting required
* Current California licensure required
* Current CPR certification required
