Job Details:

* Day shift schedule
* Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions
* Complete evaluations to ensure quality standards and address assessment areas that require attention
* Use appropriate clinical techniques and tools to establish individualized treatment plans for patients
* Provide therapy interventions using appropriate treatment protocols and techniques
* Establish individualized care and discharge plans and ensure plans are communicated to appropriate parties
* Complete comprehensive and accurate clinical documentation within established deadlines
* Supervise assigned assistants
* Serve as co-leader in interdisciplinary approach to patient groups
* Doctor of Physical Therapy (DPT) degree required if license obtained on or after January 2016 or foreign equivalent
* Minimum two years of Physical Therapy experience preferred
* Physical Therapy licensure in state of practice required
* Basic Life Support (BLS) certification from approved vendor required

This facility has been deeply connected to the community it serves since 1956. It is comprised of a 50-bed acute care medical center, level IV trauma center, level II intensive care nursery services, an extensive physical rehabilitation program, outpatient surgery and diagnostic center, and an advanced wound care center.
