Physical Therapist - Home Health

Job Details:

* Provide physical therapy services across assigned county territories including Oakland, Berkeley, Alameda, Albany, Emeryville, and Piedmont (Area #2) as well as Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, and Kensington (Area #8)
* Additional travel outside of assigned areas may be required
* Weekend coverage may be required
* Guarantee option requires a minimum productivity of 27 points per week with a 36-hour guarantee
* Must be willing to travel outside the area or work weekends to maintain weekly visit expectations under the guarantee option
* Guarantee does not apply during the first and last week of assignment, weeks with holidays, or weeks in which the traveler has time off
* Non-guarantee option allows patient refusal with no guaranteed hours and payment made on a per visit basis
* Visit types include OASIS ReCertification, OASIS RoC, Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F/up, Re-Eval, Eval Discharge, OASIS Discharge, and OASIS F/up
* Hours are calculated based on per visit conversion: 1 point equals 1.33 hours and OASIS SoC at 1.5 points equals 2 hours
* Rate covers drive and travel time, visit time, documentation time, and time spent making phone calls to patients and doctors
* Required credentials: Current California PT License, BLS certification, valid Driver's License, auto insurance, auto registration
