Opportunity:

Director of Care Coordination

Job Details:

* Lead and oversee Care Coordination, Utilization Management, and Discharge Planning functions within an acute-care environment
* BSN required; master's degree in Nursing, Health Administration, or Business Administration preferred
* Case Management certification (e.g., CCM or ACM) preferred
* Minimum 5 years of experience in an acute healthcare setting required
* Minimum 5 years of progressively responsible leadership or administrative experience in Case Management, Care Coordination, Utilization Management, or a closely related function required
* Demonstrated experience leading interdisciplinary clinical and operational programs, staff, performance improvement, and regulatory or accreditation readiness
* Advanced knowledge of case management, care coordination, utilization management, discharge planning, patient progression, and transitions of care in an acute-care environment
* Up to 36 hours of call-off available during the course of the assignment
