Opportunity:

RN - Case Manager

Job Details:

* Monday through Friday schedule from 8:00 AM to 5:00 PM with on-call and weekend rotation
* 8-week contract assignment
* Quick start position
* Provide skilled nursing and case management for assigned patients in their place of residence
* Admit patients onto services and coordinate and integrate care to ensure continuity
* Observe, assess, evaluate, and plan interventions related to patients' physical and psychosocial statuses
* Oversee care provided by Home Health Aides and CNAs and evaluate compliance with plan of care
* Conduct effective communication, information exchange, and case conferences with other disciplines and community agencies
* Provide symptom management and promote optimal health through teaching, counseling, and appropriate referrals
* Maintain accurate and complete patient records in accordance with agency standards and OASIS requirements
* Participate in team scheduling coordination and on-call rotation as needed
* Participate in quality improvement activities and in-service training
* Current RN license in the State of Montana required (BSN preferred)
* Valid driver's license, appropriate auto insurance, and reliable transportation required
* 2 years of recent acute care, home care, or hospice experience preferred including venipuncture, catheterization, wound care, and/or parenteral therapy
* HCHB experience strongly preferred
* Position may start as a Visiting RN and transition to Case Manager role
