Opportunity:

RN - Case Management

Job Details:

* Day shift position working Tuesday through Saturday from 8:00 AM to 4:30 PM
* 40 hours per week guaranteed
* No on-call responsibilities
* Minimum two years of recent case management nursing experience required
* Inpatient case management experience strongly preferred
* Experience with strike force environments such as Kaiser Permanente or HCA California preferred
* Bachelor's Degree in Nursing (BSN) preferred
* Current BLS certification required
* Active Registered Nurse (RN) license required in state of practice
* Case management certification preferred
* Lead coordination of patient care across multidisciplinary teams, monitoring appropriateness and timeliness of care
* Ensure interdisciplinary care plans align with patient clinical course, continuing care needs, and covered services
* Discuss resource utilization, treatment plans, estimated length of stay, and discharge planning with physicians
* Focus on complex patients including frequent emergency department utilizers, chronic pain patients, substance abuse patients, and homeless patients
* Coordinate patient transfers to tertiary centers including transfer of required patient information
* Manage frequent emergency department visitors through focused study reviews of previous and current admissions
* Provide oversight and collect data required for regulatory and accreditation compliance
* Act as liaison between patients, families, nurses, physicians, and multidisciplinary care teams
