Opportunity:

RN - Case Management

Job Details:

* Day shift position working 8:00 AM to 4:30 PM, Sunday through Thursday
* 40 hours per week guaranteed
* No on-call responsibilities
* Case Management RN role with a focus on inpatient case management experience
* Experience with strike force environments preferred (such as KP or HCA CA)
* Ability to handle complex discharge planning with minimal training required
* Leads coordination of patient care with multidisciplinary team, monitoring appropriateness and timeliness of care
* Ensures interdisciplinary care plan is consistent with patient's clinical course, continuing care needs, and covered services
* Discusses resource utilization, consultations, treatment plans, estimated length of stay, and discharge planning with physicians
* Focuses on complex patients including frequent emergency department utilizers, chronic pain patients, substance abuse patients, and homeless patients
* Coordinates transfer of patients to tertiary centers including transfer of patient information for continuity of care
* Manages frequent emergency department visitors by conducting focus study reviews of previous and current admissions
* Minimum two years of recent experience in case management required
* Bachelor's Degree in Nursing (BSN) preferred
* Case management certification preferred
* Current BLS certification required
* Current RN licensure in state of practice required
