Opportunity:

Home Health RN (All Inclusive BR) 8hr or 10hr shifts

Job Details:

* 8-hour or 10-hour day shifts (open to discussion)
* 40 guaranteed hours per week (hours not guaranteed during orientation week or holidays when unit is closed)
* Every other weekend rotation (open to every weekend) and up to every other holiday rotation required
* 13-week contract
* Start from the Home Health office and cover a 9-county radius in Northeast Wisconsin (average travel of 75-100 miles per day)
* 4-6 patient visits per day with travel time included in shift length
* Must have own vehicle for travel between patient homes
* No on-call or callback responsibilities
* Care for a wide variety of patients ranging from newborns to seniors following illness, new diagnosis, injury, or surgery
* Work alongside a multidisciplinary team including nursing, physical/occupational/speech therapy, social work, and CNA staff
* Comfort with hands-on skills such as IV site care, wound care, wound vac, and ostomies required
* Must be comfortable working in all home types (no special requests such as no pets or no smoking can be accommodated)
* Up to 50 pounds of lifting, pushing, and pulling during patient care may be required
* Utilize advanced technology including remote patient monitoring, 1-lead electrocardiogram capture, and video visits
* Laptop, corporate phone, and clinical bag with vital equipment and PPE provided
* Solid color scrubs or business casual dress required
* EPIC experience required; point of care documentation experience required; OASIS experience a plus but not required
* Minimum 2-3 years of home health experience required; first-time travelers accepted with a minimum of 3 years of experience
* Associate Degree or Certificate required; BSN preferred
* Required certifications: Current Wisconsin RN license or Compact, AHA BLS

This home health organization provides care to a wide variety of patients across a multi-county region in Northeast Wisconsin, supporting patients following illness, new diagnosis, injury, or surgery with the goal of helping them return to their prior level of function or better. The facility operates with a comprehensive multidisciplinary clinical team and utilizes advanced remote patient monitoring technology to closely track patient vital signs and key clinical information. Staff safety is a top priority, with the team being mindful of inclement weather and proactively steering visits away from affected areas when necessary.
