Opportunity:
Case Manager
Job Details:
* Minimum 2 years of acute care hospital Case Manager experience performing Care Coordination and Discharge Planning required
* Hospital experience is required; behavioral health-only experience does not qualify
* Travel experience required; first-time travelers accepted and will be screened by clinical team
* Patient ratio of 1:20
* Weekend rotation required to meet department needs
* No floating required
* California state license required; pending license accepted but must be active upon start date
* Scheduling type is variable; other scheduling arrangements based on department needs
* Pre-approved time off limited to no more than 7 days
* Patients served include adolescents, adults, and geriatrics
* Experience with admission criteria (InterQual), assessing patients, care coordination, and discharge planning required
* Knowledge of drug/alcohol abuse treatment options, patients experiencing homelessness, mental health history or crisis, advance directives, POLST, and protective services processes required
* Ability to organize and participate in family meetings and contribute to treatment plan development
* Experience with HMOs, IPAs, and similar organizations required
* Must be able to independently arrange DME, SNF placement, and ambulance transport
* Proficiency with Epic charting system required
* Proficiency in Microsoft applications required
* Computerized documentation experience required
* Scrubs or business attire accepted
* Skills required include: acute hospital care, care coordination, CMS, discharge planning, DRG, plan of care, and InterQual criteria
* Interdisciplinary support includes social services and transportation
