RN - Case Management
Job Details:
* Weekend rotation required to meet department needs
* Patient ratio of 1:20
* No floating required
* Scheduling type is variable based on department needs
* Orientation hours: 40
* 2 years of acute care hospital Case Management experience required performing Care Coordination and Discharge Planning
* Travel experience required
* California state license required; pending license accepted but must be active upon start date
* Scrubs or business attire required
* Must treat patients across age groups including adolescents, adults, and geriatrics
* Experience with assessing patients, care coordination, and discharge planning required
* Knowledge of drug/alcohol abuse treatment options, homelessness resources, mental health crisis support, advance directives, POLST, and protective services process required
* Ability to organize and participate in family meetings and contribute to treatment plan development
* Experience with HMOs and IPAs required
* Must be able to independently arrange DME, SNF placements, and ambulance transport
* Experience with InterQual criteria and admission criteria required
* Proficiency in Epic charting system required
* Proficiency in Microsoft applications required
* Computerized documentation required
* Skills required: Acute Hospital, Care Coordination, CMS, Discharge Planning, DRG, Plan of Care, and InterQual Criteria
* Interdisciplinary support includes social services and transportation
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* Pre-approved time off limited to no more than 7 days
* 2 professional references required covering 1 year within the last 3 years
