Job Details:

* 2 years of acute hospital case management experience required
* Travel experience required; first-time travelers will not be considered
* Patient ratio of 1:35, with a case load of up to 25 patients daily
* Required to monitor up to 2 units on weekends with a focus on discharge planning
* Every other weekend required; must be available for holiday coverage including 3 of 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
* California state license preferred; pending license accepted
* RTO restrictions: no more than 7 days
* Charting system: Epic (required)
* Scrubs or business casual attire required
* Certification required: MSW
* Must be proficient in performing both care coordination and discharge planning roles
* Key responsibilities include biopsychosocial assessments, discharge planning, care coordination, family meetings, treatment plan participation, and working with multidisciplinary teams
* Knowledge of drug/alcohol abuse treatment options, patients experiencing homelessness, mental health crisis, and POLST required
* Computerized documentation required; Epic charting system experience required
* Patient age groups include neonates, infants, adolescents, pediatrics, adults, and geriatrics
* Travelers may be expected to float between nearby sister facilities; refusal to float may result in termination or forfeiture of guaranteed hours

The facility supports interdisciplinary care through IV teams, physical therapy, respiratory services, interpretation services, phlebotomy, radiology, social services, rapid response teams, pharmacy, transportation, and unit secretaries.
