Opportunity:

RN - Case Management

Job Details:

* Monday through Friday schedule from 8:00 AM to 4:30 PM with rotating weekends
* No on-call requirements
* Patient ratio of 1:25, varies
* May float to any unit as needed
* Work alongside RNs, Social Workers, and CMAs
* Document using Meditech, Midas, and Wellsky systems
* Recent acute care discharge planning experience required
* Business casual dress code required

This medical center is a large acute care hospital with 590 beds, offering a wide range of inpatient and outpatient services to the community. The Case Management unit works collaboratively with a multidisciplinary team to coordinate patient care and discharge planning. The facility utilizes multiple documentation systems to support efficient and effective patient care management.
