Opportunity:

RN - Case Management

Job Details:

* Seasoned travelers only - first-time travelers not accepted
* Minimum 2 years of acute care case management and discharge planning experience required; behavioral health experience does not apply
* Must have hospital experience
* California state license required; pending license accepted but must be active upon start date
* Patient ratios: 1:25 on days, 1:25 on weekends
* Weekend rotation required (every other weekend)
* No floating requirement
* Charting system: Epic
* Business attire or scrubs required
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* Pre-approved time off limited to no more than 7 days
* Orientation: 40 hours
* Patient age groups served: neonates, adolescents, adults, and geriatrics
* Must have experience assessing patients, care coordination, discharge planning, and utilization review
* Must be able to organize and participate in family meetings and contribute to treatment plan development
* Experience with HMOs, IPAs, and similar managed care organizations required
* Knowledge of adoption/foster care, drug and alcohol abuse treatment options, patients experiencing homelessness, mental health history or crisis, advance directives, POLST, and protective services processes required
* Must be competent and able to independently arrange DME, SNF placement, and ambulance transport
* Interdisciplinary support includes physical therapy and social services
* Computerized documentation required; Epic charting system used
* Travelers may be expected to float between nearby sister facilities; refusal to float may result in termination and/or forfeiture of guaranteed hours

This acute care facility provides comprehensive inpatient services across a broad range of patient populations including neonates, adolescents, adults, and geriatrics. The facility operates with an interdisciplinary care model that incorporates physical therapy and social services to support patient outcomes. The organization utilizes Epic for electronic documentation and maintains high standards for case management, discharge planning, and utilization review practices.
