Job Details:

* 2 years of acute hospital case management experience required
* Travel experience required
* Every other weekend required with availability for holiday coverage
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* Patient ratio of 1:35, with a case load of up to 25 patients daily
* Required to monitor up to 2 units on weekends with a focus on discharge planning
* Performs both care coordination and discharge planning roles
* Utilizes Epic charting system (required)
* Computerized documentation required
* Scrubs or business casual attire required
* California state license preferred; pending license accepted
* Required certification: MSW
* Must have knowledge of drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, and POLST
* Must be able to organize and participate in family meetings and contribute to treatment plan development
* Performs biopsychosocial assessments, discharge planning, care coordination, and prior authorizations
* Works with multidisciplinary team to ensure continuity of care
* Serves patient age groups including neonates, infants, adolescents, pediatrics, adults, and geriatrics
* Interdisciplinary support available including IV teams, physical therapy, respiratory services, pharmacy, and rapid response teams
* RTO restrictions: no more than 7 days
* May be expected to float between sister facilities nearby; refusal to float may result in termination of assignment and/or forfeiture of guaranteed hours
