Opportunity:

RN - Case Manager

Job Details:

* Minimum 2 years of acute care case manager experience required
* California case manager experience required (6 months acceptable)
* Active California RN license required - no pending or temporary licenses accepted
* Epic charting system experience highly preferred
* Required certifications: BLS
* Patient ratio of 15 patients per day
* No weekend shifts required
* No floating required
* No on-call required
* No scheduling requests can be accommodated - must remain flexible
* Serves patient age groups including adolescents, adults, and geriatrics
* Skills required include discharge planning, care coordination, utilization of InterQual criteria, utilization of Milliman guidelines, plan of care, ICD-10 coding, DRG knowledge, and HIPAA guidelines
* Flu vaccination required - no exemptions
* 2 supervisor or higher references from within the past year required at time of submission

Facility:

This facility is a large, well-established acute care hospital serving a diverse patient population including adolescents, adults, and geriatrics. With a daily census of 255 and interdisciplinary support including social services and transportation, the facility provides a collaborative environment for case managers. The organization maintains high compliance and credentialing standards to ensure quality patient care across all departments.
