Opportunity:
Care Manager
Job Details:
* Day shift schedule with a patient ratio of 1:25
* Weekend requirement is every other weekend, Saturday and Sunday, with a day off during the weekday
* No floating requirement
* No on-call requirement
* 40-hour orientation period
* Dress code: Business attire or scrubs
* Minimum 2 years of acute medical Care Manager experience performing Care Coordination and Discharge Planning required
* Travel experience required; first-time travelers will be screened
* California state license required; pending license accepted but must be licensed upon start date
* Epic charting system experience required
* Experience with HMOs, IPAs, and similar organizations required
* Experience assessing patients, performing Care Coordination, and Discharge Planning required
* Computerized documentation experience required
* Must be knowledgeable in CMS, CPT coding and billing, DRG, HIPAA guidelines, and evidence-based medical necessity guidelines
* Skills required include: Acute Hospital, Admission Criteria, Care Coordination, Disease Management, Discharge Planning, Needs Assessment/Order DME, Plan of Care, and ICU/MS/SDU/PCU/IMC/Obs experience
* Serves adult and geriatric patient populations
* Case load of up to 25 patients
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* RTO restricted to no more than 7 days; all RTO requests must be submitted upfront during submittal
* 2 references required, covering 1 year within the last 3 years, due during compliance process and prior to start

This is an acute care facility offering comprehensive medical services to adult and geriatric patient populations. The facility supports interdisciplinary care coordination and discharge planning and utilizes Epic as its primary charting system. The organization maintains high standards for patient care and requires experienced professionals who are well-versed in care management, medical necessity guidelines, and regulatory compliance.
